New Client Forms
Completing your new client forms on-line will save time and make check-in easier when you arrive at the office. Please take a moment to read through our Client Bill of Rights and Responsibilities. We look forward to meeting you and your pet!
Free New Client Exam
If this your first visit to our hospital we offer a free exam with one of our veterinarians for wellness care or another non-urgent appointment. The exam and consultation is our gift to you, but a purchase of other services or products is required (invoice total cannot be $0.00). You will be financially responsible for additional services such as vaccines, diagnostic tests, medical procedures, or other services or products that you elect during the appointment.
Appointments must be made in advance. Walk-ins, work-ins, urgent care, and emergency visits are excluded. Offer is good for a standard physical exam on a dog or cat and does not include specialty exams such as exotic pets, breeding exams, or dermatology visits with our veterinary dermatologist, Dr. Michael Groh.
To receive the complimentary new client exam, mention it when you make an appointment and complete your new client forms on-line using the link above. Limit one free exam per client for your first visit only.
Free New Client Second Opinion
If this is your first visit to our hospital and you are seeking a second opinion on an illness or problem, we want to help you find answers. We have a team of doctors who share experience and expertise and work together on tough cases.
We will waive the exam and consultation fee for new clients on your first visit for a second opinion. You will be financially responsible for any diagnostic tests or treatments you elect to do based upon the exam, but the exam and consultation is our gift to you. Free new client second opinion exams are available with any of our doctors except our veterinary dermatologist, Dr. Michael Groh.
We want our doctors to have adequate time set aside to focus on 2nd opinions, which are usually more complex than typical cases. Therefore free new client second opinion appointments must be scheduled for an early time slot in the day. Morning or early afternoon appointments are preferred.
To receive a complimentary new client 2nd opinion exam, complete the on-line new client forms and upload medical records from previous veterinarians who have treated the problem. If you do not have a copy of the records, please have your veterinarian send the records over prior to your appointment with us.
Please call 816-229-1544 to make an appointment, and tell our receptionist that you are a new client wanting a second opinion so they allow extra time for your visit. Limit one free exam per client for your first visit only.
